In an effort to maintain a safe environment for our campers, all volunteers must consent to a background check being performed. In the coming weeks, as your application is processed, you will receive an email from Ministry Safe asking you to submit further information specifically for the Background Screening. Please be watching your email inbox. Once the email is sent, you will have 14 days to complete the form before the link expires. Without a completed background screening, you will not be approved to volunteer as camp staff.
We encourage you to find other arrangements for your children during the week of camp. If other arrangements are not possible, children under the age of ten are permitted to attend, pending youth camp executive team approval. Without written consent from the Youth President, your children (age 0-10) are not permitted to accompany you to camp . The following information must be provided: name and age. Parents/Guardians must sign a liability waiver for each child. Children 11 and up are not permitted to attend youth camp unless they are registered campers.

Code of Christian Conduct
Please read carefully. By signing below, ministry community member acknowledges an understanding that this ministry only hires staff members and volunteers who agree to abide by its Guidance Statements and Code of Conduct which is rooted in its sincerely held religious beliefs and which this ministry believes reflects a relationship representative of a walk with Christ.
In addition, by signing below, ministry community member acknowledges they have read, understood, and agree to abide by these Guidance Statements and Code of Conduct and that this ministry reserves the right to discipline said community member (up to and including termination from the community) for any action(s) in violation of these Guidance Statements and Code of Conduct.
1. Policy Inclusions. This ministry’s code of conduct is rooted in its sincerely held religious beliefs and is consistent with the following: The Ten Commandments (Exodus 20:2-17) The Great Commandments as stated by Jesus Christ (Matt. 22:37-40) 2. Personal Conduct. All persons connected with this ministry community (e.g. employees, volunteers, board members, students, parents of students, etc.) shall hereinafter be referred to as ministry community members. All such ministry community members must conduct their personal affairs so there can be no opportunity for unfavorable reflections upon the Christian beliefs and mission of the ministry, either expressed or implied. The use of common sense, good ethical standards and discretion will guide all who are called into community with the ministry in proper conduct. Failure to maintain reasonable standards is subject to discipline, up to and including termination, from the ministry community. It is expected that all members of our ministry community will conduct themselves in a manner consistent with biblical standards, values, and character. 3. Rules of Conduct. In every ministry where many people interact on a regular basis, some specific rules and policies are necessary to establish acceptable standards of conduct, assure fair treatment of all members of the community, and enhance the smooth operation of the ministry. Ministry community members are expected to become familiar with and abide by the standards outlined in this policy. The purpose of these rules is to maintain a community environment that protects the safety and dignity of each community member without placing unreasonable restrictions on anyone. Ministry community members are expected to model appropriate behavior and conduct both during and outside of district functions and model relationships that demonstrate a growing Christ-likeness manifested in a lifestyle that serves and gives itself to reconcile others. Ministry community members are also expected to model appropriate language on and off-campus and model speech that demonstrates a growing Christ-likeness (Ephesians 5:4). Ministry community members shall maintain appropriate attitudes of concern for others. Problems concerning ministry community members’ roles, relationships, and professional conduct should first be handled directly with the person involved. If a satisfactory resolution cannot be concluded, the matter should move up the relevant chain of responsibility. Specifically, ministry community members shall respect the integrity and confidences of other community members and those outside of our community attending any of our ministry’s functions. Ministry community members are expected to operate within their respective roles. In summary, Christian ethics demand that ministry community members act in love and integrity, in confidentiality, and in alignment with the mission/purpose of this ministry. 4. Inappropriate Conduct. Violation of ministry rules and policies may result in an oral warning, a written warning, and/or termination of a member’s role in the ministry community. There is no requirement that discipline be progressive or that a warning be given prior to demotion or any other disciplinary action, including discharge from the ministry community. Set forth below are some examples of misconduct which will not be tolerated by the ministry. This list is not exhaustive and examples are not listed in order of seriousness. Falsification of personnel and/or ministry records. Dating, or otherwise becoming romantically involved with, anyone under legal age. Engaging in any activity which a ministry community member knows, or has reason to know, will adversely affect the mission of the ministry. Engaging in sexual acts and/or relationships outside the confines of biblical marriage between one man and one woman. Deliberate damage or destruction of any ministry property or the property of any ministry community member. Engaging in criminal conduct. Insubordination, including but not limited to, failure or refusal to obey the orders or instructions of a supervisor, district leader, or other ministry leader, or the use of abusive or threatening language toward a supervisor, district leader, or other ministry leader. Using abusive language at any time during district events. Failure to notify a supervisor when unable to report to work. Sleeping on the job. Wearing unprofessional or inappropriate styles of dress (including dressing in such a way as to willfully reject one’s sex assigned at birth (Gen. 1:27)). Violation of any safety, health, security or ministry policies, rules or procedures. Committing a fraudulent act or a breach of trust under any circumstances. Unlawful harassment including harassment of a sexual nature (with the understanding that this ministry reserves the right to operate according to its sincerely held religious beliefs about biblical marriage and sexuality and therefore does not define “harassment” as including any enforcement of, or adherence to, its biblical principles and expectations in the areas of marriage, sexuality, dress, and discipline). Engaging in behavior that suggests a willful violation of the religious beliefs and practices of the ministry. Sex-specific sleeping areas, changing areas, restrooms, and showers are to be used by members of the designated biological sex only. If a ministry community member has any doubt about whether certain conduct will constitute behavior that suggests a willful violation of the religious beliefs and practices of the ministry, the community member should ask an appropriate ministry leader.

The following statements of faith provide a summary of biblical doctrines that are affirmed by the United Pentecostal Church Oklahoma District: About God: There is one God, who has revealed Himself as our Father, in His Son Jesus Christ, and as the Holy Spirit. Jesus Christ is God manifested in flesh. He is both God and man. (See Deuteronomy 6:4; Ephesians 4:4-6; Colossians 2:9; I Timothy 3:16.) About the Bible: The Bible is the infallible Word of God and the authority for salvation and Christian living. (See II Timothy 3:15-17.) About Sin and Salvation: Everyone has sinned and needs salvation. Salvation comes by grace through faith based on the atoning sacrifice of Jesus Christ. (See Romans 3:23-25; 6:23; Ephesians 2:8-9.) About the Gospel: The saving gospel is the good news that Jesus died for our sins, was buried, and rose again. We obey the gospel (II Thessalonians 1:8; I Peter 4:17) by repentance (death to sin), water baptism in the name of Jesus Christ (burial), and receiving the gift of the Holy Spirit (resurrection). (See I Corinthians 15:1-4; Acts 2:4, 37-39; Romans 6:3-4.) About Our Fundamental Doctrine: The basic and fundamental doctrine of this District is the Bible standard of full salvation, which is repentance, baptism in water by immersion in the name of the Lord Jesus Christ for the remission of sins, and the baptism of the Holy Ghost with the initial sign of speaking with other tongues as the Spirit gives utterance. (See John 3:5, Acts 2:38.) About Christian Living: As Christians we are to love God and others. We should live a holy life inwardly and outwardly, and worship God joyfully. The supernatural gifts of the Spirit, including healing, are for the church today. (See Mark 12:28-31; II Corinthians 7:1; Hebrews 12:14; I Corinthians 12:8-10.) About Sexuality: We believe that God wonderfully and immutably creates each person as male or female. These two distinct, complementary sexes together reflect the image and nature of God (Gen. 1:26-27). Rejection of one’s biological sex is a rejection of the image of God within that person. About Marriage: We believe the term “marriage” has only one meaning: the uniting of one man and one woman in a single, exclusive union, as delineated in Scripture (Gen. 2:18-25). We believe that marriage between one man and one woman, for life, uniquely reflects Christ’s relationship with His Church (Eph. 5:21-33). We believe that God intends sexual intimacy to occur only between a man and a woman who are married to each other (1 Cor. 6:18; 7:2-5; Heb. 13:4). We believe that God has commanded that no intimate sexual activity be engaged in outside of a marriage between a man and a woman. We believe that any form of sexual immorality (including adultery, fornication, cohabitation, homosexual behavior, rejection of one’s biological sex or use of pornography) is sinful and offensive to God (Matt. 15:18-20; 1 Cor. 6:9-10). We believe that God offers redemption and restoration to all who confess and forsake their sin, seeking His mercy and forgiveness through Jesus Christ (Acts 3:19-21; Rom 10:9-10; 1 Cor 6:9-11). We believe that every person must be afforded compassion, love, kindness, respect, and dignity (Mark 12:28-31; Luke 6:31). Hateful and harassing behavior or attitudes directed toward any individual are to be repudiated and are not in accord with Scripture nor the doctrines of the ministry. About the Sanctity of Human Life: We believe that all human life is sacred and created by God in His image. Human life is of inestimable worth in all its dimensions, including pre-born babies, the aged, the physically or mentally challenged, and every other stage or condition from conception through natural death. We are therefore called to defend, protect, and value all human life (Ps. 139). About Final Authority for Matters of Belief and Conduct: The statements of faith do not exhaust the extent of our beliefs. The Bible itself, as the inspired and infallible Word of God that speaks with final authority concerning truth, morality, and the proper conduct of mankind, is the sole and final source of all that we believe. For purposes of the United Pentecostal Church Oklahoma District’s faith, doctrine, practice, policy, and discipline, our District Superintendent is the United Pentecostal Church Oklahoma District’s final interpretive authority on the Bible’s application.

Participant Waiver and Liability Release
Please read carefully. This Agreement affects your legal rights and is legally binding. By signing this Agreement, you are releasing the United Pentecostal Church International (UPCI), The Oklahoma District of the UPCI, The Oklahoma District Campground, The Oklahoma Children’s Ministry of the UPCI, the Oklahoma Youth Department of the UPCI, and all pastors, ministers, officers, directors, employees, volunteers, agents, representatives, and insurers (hereinafter the “Oklahoma District”) from all liability and forever giving up any claims thereof.
General - I acknowledge and agree that there are risks inherent in attendance and participation in the Oklahoma District camp, which may result in various types of injury or damage, including but not limited to, the following: sickness, exposure to infectious/communicable disease, bodily injury, death, emotional injury, personal injury, property damage, and financial damage. In consideration for mine and/or a minor child’s opportunity to attend and/or participate in the Oklahoma District camp, I, on behalf of myself and/or the minor child, acknowledge and accept the risks associated with participation and attendance. On behalf of myself and/or a minor child, I knowingly accept personal financial responsibility for any injury or other loss sustained during the attendance and participation in the Oklahoma District camp. I further accept personal financial responsibility for any medical treatment rendered to the participant that is authorized by the Oklahoma district, its agents, employees, volunteers, or other representatives. Further, I hereby release the Oklahoma District of all liability for any property damage, injury, loss, illness, or death. I further agree to indemnify, defend, and hold harmless the Oklahoma District, its employees, leaders, owners, agents, and representatives from any and all causes of action, claims, demands, losses, or costs of any nature arising out of or in any way related to my or the minor child’s attendance or participation in the Oklahoma District Camp, including but in no way limited to damages or liability from damage to property, bodily injury, personal injury, emotional injury, illness, permanent disability, and/or death, as well as medical expenses and other costs for myself or the minor child.

Consent and Certification
The information contained in this application is correct to the best of my knowledge. I authorize any reference or churches listed in the application to provide any information (including opinions) that they may have regarding my character and suitability for youth work. I understand a criminal record check may be conducted as a result of this application. In consideration of the receipt and evaluation of this application by the Oklahoma District Youth Department, I hereby release any individual, church, youth organization, charity, employer, reference, or any other person or organization, including record custodians, both collectively or individually, from any and all liability for damages of whatever kind of nature which may at the time result to me, my heirs, my family, on account of compliance or any attempts to comply, with this authorization. I waive any right that I may have to inspect any information provided about me by any person or organization identified by me in this application. Individuals using the UPCOK facilities agree to submit any legal dispute with the UPCOK for mediation before a mutually agreed-upon mediator, or if none can be agreed upon, one selected by Peacemaker Ministries. Lawsuits between believers, or threats of lawsuits between believers, are a matter of grave concern for the church, are contrary to biblical and church teaching, and mediation is an effort to resolve disputes in a biblical fashion (1 Cor. 6: 1-7). Mediation will be governed by the Rules of Procedure for Christian Conciliation of the Institute for Christian Conciliation (ICC Rules), unless modified as stipulated by the parties. In particular, subject to the more detailed provisions of the ICC Rules, mediator(s) will attempt to assist us in reaching a voluntary settlement of any disputes through mediation. The confidentiality of the mediation process will be protected and these matters will not be discussed with people who do not have a necessary interest in them. If settlement can be agreed upon, the conciliators may, at their discretion, issue an advisory opinion. Neither the opinion, nor any communications exchanged in the mediation process, will be admissible for any purpose in any subsequent legal proceeding. I also understand that my personal insurance (if applicable) is primary coverage and the Youth Camp’s insurance is secondary. I agree to and hereby release from all liability, personal and/or property, and hold harmless the Oklahoma District U.P.C., its subsidiaries, trustees, employees, agents, sponsors and volunteers from all legal responsibility, including claims, demands and lawsuits resulting from or related to any incidents arising from or connected in any manner with the Oklahoma District U.P.C. Youth Camp, including, but not limited to, liability, damages and legal fees or costs caused by or related to the negligence or intentional act of the Oklahoma District U.P.C., its subsidiaries, its trustees, agents, sponsors or volunteers. Should my application be accepted, I agree to be bound by the Bylaws and Youth Camp Policies of the Oklahoma District United Pentecostal Church, to comply with all Youth Camp Regulations and fulfill my job descriptions given by the Youth Camp Executive Committee and refrain from unscriptural conduct in the performance of my services on behalf of the district, I further state that I HAVE CAREFULLY READ THE FOREGOING RELEASE AND KNOW THE CONTENTS THEREOF AND I SIGN THIS RELEASE AS MY OWN FREE ACT. Section 1: Policy Whereas the world is trying to invade our ranks, and due to some young people being new in the church, and to help maintain a godly standard of holiness, we as a District see a need to hold to godly standards of holiness. Be it understood that we DO NOT consider these Dress Codes the maximum holiness standard, but only the minimum for the purpose of conducting a Youth Camp. Be it further required that a copy of the “Dress Codes” be made available to all of our pastors and parents and that they be responsible for informing all campers of said code and enforcement. Be it further required that this dress code be enforced at Youth Camp by the Youth Camp Executive Committee. Enforcement procedure will involve the following: After a complaint has been made by Youth Camp faculty or Youth Camp help, such as dorm supervisors, camp patrol, etc., the youth first be admonished to comply with the code. If the code is repeatedly or continuously violated, the pastor of the youth is to be advised to notify the parents and the youth is to be sent home. Section 2: Dress Codes (Campers / Camp Staff) Boys’ hair must not be any longer than would reach a collared shirt in the back or over the ears. Boy’s sideburns are to be no longer than the bottom of the ear. No facial hair is allowed on boys. Must be clean-shaven upon arrival to campgrounds. No pajamas or short pants are allowed outside of the dorms. Hats should not be worn by males in services. No plain white tee shirts may be worn by boys or girls as outer apparel. Jewelry of any kind is prohibited on the campgrounds (necklaces, earrings, wrist or ankle bracelets, etc). A wedding band is permitted to be worn by married staff members. (1 Timothy 2:9) Make-up or nail polish, including “French Nails,” are not allowed to be worn at any time. (1 Peter 3:3-4) Shirts, blouses, and dresses are to have at least a standard short sleeve, which is defined as coming at least midway between the shoulder and elbow. Girls’ dress must be modest, not revealing or exposing the body immodestly by slits, low necklines, or thin, see-through material. All dresses should at minimum cover the collar bone to maintain modesty. (1 Peter 1:15) Dresses and skirts must be to the knee when sitting down. No slits should be above the knee. (1 Peter 1:15) All shirts worn by both boys and girls should be a minimum of waist-length when hands are raised. No sleeveless or cap sleeve dresses or blouses. No steel cleats of any type shall be worn at camp. Clothing that explicitly or implicitly promotes racism, sexism, sexual actions, profanity, tobacco, drugs, alcohol, or hatred toward a certain group of people will not be allowed. (Colossians 3:8, Ephesians 4:29, Ephesians 5:4)

All camper’s vehicle keys must be turned in to the administration office upon arrival and campers cannot use their vehicles during camp. Campers are not permitted to leave the grounds without permission from the Camp Executive Committee. Campers are only permitted to be in designated approved areas of the camp and are required to attend and be prompt to all classes, meals, recreational activities and services, unless excused by the administrator. Boys are not permitted to enter the area of the girls’ dormitories and the girls are not permitted to enter or visit anyone in the boys’ section. (Hebrews 13:17) Woods, creek, and lagoon are off limits to campers. Athletic equipment and facilities are under the control of the Recreational Directors and may be used as specified by them. Every camper, counselor and staff member is responsible for their own bedding, clothing, luggage, and personal belongings. It is not encouraged for campers and camp staff members to bring “valuables” to camp as they may get lost, broken or stolen. All should give respect to the camp grounds, buildings and facilities. Individuals defacing camp property or the natural surroundings must pay the damage incurred. (Luke 6:31) Absolute quiet and attention is expected from every camper during classes and services. Enter the Tabernacle with a spirit of reverence and leave as orderly and quietly as possible. These services should be high moments of inspiration. (Leviticus 19:30) Campers and counselors are required to stay in their dorms each night from the sounding of “Lights Out” until breakfast the following morning, except for special emergencies. No lights or noise is permitted in the dorms during sleeping hours. It is expected everyone dress modestly and in accordance with the Dress Code at all times. (Romans 12:1) All campers are to remain in their assigned areas for sleeping. No “switching” is allowed without previous consent of both counselors involved. Boys and girls will not be allowed to sit together during classes or services. All campers, staff and counselors are required to wear their identification badges at all times. A camper is expected to get permission from security before leaving a class or night service for any reason. Campers may not leave the night service until the “Dismissed” sign is posted. Campers are prohibited from the use of any motorized vehicles without permission from the Camp Executive Committee. The general standard of behavior shall be such as is becoming to Christian Youth, as defined by the Statement of Faith and Code of Christian Conduct. No one will be allowed to stay off the grounds and attend camp without special permission by the Camp Executive Committee. While conversation and fellowship are allowed, there shall be no dating during camp. Boys shall not escort the girls to their dormitory or vise versa. Public display of affection of any kind is strictly prohibited. No weapons, including but not limited to knives, guns, laser pointers or explosives (including fireworks) are permitted at camp. The use of tobacco, drugs or alcohol in any form will not be allowed on the camp grounds. All prescription drugs must be turned in to camp nurse. (I Corinthians 6:19) No violations of sexual morality as defined in the Statement of Faith, including but not limited to pornography or sexual activity will be allowed on the grounds. (I Corinthians 6:18-20) Cell phones are allowed at camp with the following stipulations: Streaming services (Netflix, Hulu, YouTube, etc.) are prohibited. Ear buds and headphones of any kind are prohibited as well. Photography and videography of any kind (including video messaging) is not allowed in the dormitories and bathrooms. Campers found guilty of theft or malicious damage to any articles by confession or evidence, shall forfeit all rights and must leave the campgrounds. The responsible party must make restitution for any damages. (Exodus 20:15, Matthew 22:39) All campers who attend the camp are subject to the rules and dress codes of the camp, and are not allowed to leave the camp without special permission, until they have been officially withdrawn or been dismissed from the camp.

By registering for and attending this event, I acknowledge and consent on behalf of myself and/or my minor child to the possibility that photographs, video recordings, and other media may be taken during camp activities. I grant permission for the use of mine or my child’s image, likeness, and/or voice to be used by the Oklahoma District United Pentecostal Church and its affiliated ministries for any lawful purpose, including but not limited to promotional, informational, and ministry purposes, including but not limited to use in print materials, websites, and social media platforms.
