In an effort to maintain a safe environment for our campers, all volunteers must consent to a background check being performed.
We encourage you to find other arrangements for your children during the week of camp. If other arrangements are not possible, children under the age of ten are permitted to attend, pending youth camp executive team approval . The following information must be provided: name and age. Parents/Guardians must sign a liability waiver for each child. Children 11 and up are not permitted to attend youth camp unless they are registered campers.

Code of Christian Conduct
Please read carefully. By signing below, ministry community member acknowledges an understanding that this ministry only hires staff members and volunteers who agree to abide by its Guidance Statements and Code of Conduct which is rooted in its sincerely held religious beliefs and which this ministry believes reflects a relationship representative of a walk with Christ.
In addition, by signing below, ministry community member acknowledges they have read, understood, and agree to abide by these Guidance Statements and Code of Conduct and that this ministry reserves the right to discipline said community member (up to and including termination from the community) for any action(s) in violation of these Guidance Statements and Code of Conduct.
1. Policy Inclusions. This ministry’s code of conduct is rooted in its sincerely held religious beliefs and is consistent with the following: The Ten Commandments (Exodus 20:2-17) The Great Commandments as stated by Jesus Christ (Matt. 22:37-40) 2. Personal Conduct. All persons connected with this ministry community (e.g. employees, volunteers, board members, students, parents of students, etc.) shall hereinafter be referred to as ministry community members. All such ministry community members must conduct their personal affairs so there can be no opportunity for unfavorable reflections upon the Christian beliefs and mission of the ministry, either expressed or implied. The use of common sense, good ethical standards and discretion will guide all who are called into community with the ministry in proper conduct. Failure to maintain reasonable standards is subject to discipline, up to and including termination, from the ministry community. It is expected that all members of our ministry community will conduct themselves in a manner consistent with biblical standards, values, and character. 3. Rules of Conduct. In every ministry where many people interact on a regular basis, some specific rules and policies are necessary to establish acceptable standards of conduct, assure fair treatment of all members of the community, and enhance the smooth operation of the ministry. Ministry community members are expected to become familiar with and abide by the standards outlined in this policy. The purpose of these rules is to maintain a community environment that protects the safety and dignity of each community member without placing unreasonable restrictions on anyone. Ministry community members are expected to model appropriate behavior and conduct both during and outside of district functions and model relationships that demonstrate a growing Christ-likeness manifested in a lifestyle that serves and gives itself to reconcile others. Ministry community members are also expected to model appropriate language on and off-campus and model speech that demonstrates a growing Christ-likeness (Ephesians 5:4). Ministry community members shall maintain appropriate attitudes of concern for others. Problems concerning ministry community members’ roles, relationships, and professional conduct should first be handled directly with the person involved. If a satisfactory resolution cannot be concluded, the matter should move up the relevant chain of responsibility. Specifically, ministry community members shall respect the integrity and confidences of other community members and those outside of our community attending any of our ministry’s functions. Ministry community members are expected to operate within their respective roles. In summary, Christian ethics demand that ministry community members act in love and integrity, in confidentiality, and in alignment with the mission/purpose of this ministry. 4. Inappropriate Conduct. Violation of ministry rules and policies may result in an oral warning, a written warning, and/or termination of a member’s role in the ministry community. There is no requirement that discipline be progressive or that a warning be given prior to demotion or any other disciplinary action, including discharge from the ministry community. Set forth below are some examples of misconduct which will not be tolerated by the ministry. This list is not exhaustive and examples are not listed in order of seriousness. Falsification of personnel and/or ministry records. Dating, or otherwise becoming romantically involved with, anyone under legal age. Engaging in any activity which a ministry community member knows, or has reason to know, will adversely affect the mission of the ministry. Engaging in sexual acts and/or relationships outside the confines of biblical marriage between one man and one woman. Deliberate damage or destruction of any ministry property or the property of any ministry community member. Engaging in criminal conduct. Insubordination, including but not limited to, failure or refusal to obey the orders or instructions of a supervisor, district leader, or other ministry leader, or the use of abusive or threatening language toward a supervisor, district leader, or other ministry leader. Using abusive language at any time during district events. Failure to notify a supervisor when unable to report to work. Sleeping on the job. Wearing unprofessional or inappropriate styles of dress (including dressing in such a way as to willfully reject one’s sex assigned at birth (Gen. 1:27)). Violation of any safety, health, security or ministry policies, rules or procedures. Committing a fraudulent act or a breach of trust under any circumstances. Unlawful harassment including harassment of a sexual nature (with the understanding that this ministry reserves the right to operate according to its sincerely held religious beliefs about biblical marriage and sexuality and therefore does not define “harassment” as including any enforcement of, or adherence to, its biblical principles and expectations in the areas of marriage, sexuality, dress, and discipline). Engaging in behavior that suggests a willful violation of the religious beliefs and practices of the ministry. Sex-specific sleeping areas, changing areas, restrooms, and showers are to be used by members of the designated biological sex only. If a ministry community member has any doubt about whether certain conduct will constitute behavior that suggests a willful violation of the religious beliefs and practices of the ministry, the community member should ask an appropriate ministry leader.

The following statements of faith provide a summary of biblical doctrines that are affirmed by the United Pentecostal Church Oklahoma District: About God: There is one God, who has revealed Himself as our Father, in His Son Jesus Christ, and as the Holy Spirit. Jesus Christ is God manifested in flesh. He is both God and man. (See Deuteronomy 6:4; Ephesians 4:4-6; Colossians 2:9; I Timothy 3:16.) About the Bible: The Bible is the infallible Word of God and the authority for salvation and Christian living. (See II Timothy 3:15-17.) About Sin and Salvation: Everyone has sinned and needs salvation. Salvation comes by grace through faith based on the atoning sacrifice of Jesus Christ. (See Romans 3:23-25; 6:23; Ephesians 2:8-9.) About the Gospel: The saving gospel is the good news that Jesus died for our sins, was buried, and rose again. We obey the gospel (II Thessalonians 1:8; I Peter 4:17) by repentance (death to sin), water baptism in the name of Jesus Christ (burial), and receiving the gift of the Holy Spirit (resurrection). (See I Corinthians 15:1-4; Acts 2:4, 37-39; Romans 6:3-4.) About Our Fundamental Doctrine: The basic and fundamental doctrine of this District is the Bible standard of full salvation, which is repentance, baptism in water by immersion in the name of the Lord Jesus Christ for the remission of sins, and the baptism of the Holy Ghost with the initial sign of speaking with other tongues as the Spirit gives utterance. (See John 3:5, Acts 2:38.) About Christian Living: As Christians we are to love God and others. We should live a holy life inwardly and outwardly, and worship God joyfully. The supernatural gifts of the Spirit, including healing, are for the church today. (See Mark 12:28-31; II Corinthians 7:1; Hebrews 12:14; I Corinthians 12:8-10.) About Sexuality: We believe that God wonderfully and immutably creates each person as male or female. These two distinct, complementary sexes together reflect the image and nature of God (Gen. 1:26-27). Rejection of one’s biological sex is a rejection of the image of God within that person. About Marriage: We believe the term “marriage” has only one meaning: the uniting of one man and one woman in a single, exclusive union, as delineated in Scripture (Gen. 2:18-25). We believe that marriage between one man and one woman, for life, uniquely reflects Christ’s relationship with His Church (Eph. 5:21-33). We believe that God intends sexual intimacy to occur only between a man and a woman who are married to each other (1 Cor. 6:18; 7:2-5; Heb. 13:4). We believe that God has commanded that no intimate sexual activity be engaged in outside of a marriage between a man and a woman. We believe that any form of sexual immorality (including adultery, fornication, cohabitation, homosexual behavior, rejection of one’s biological sex or use of pornography) is sinful and offensive to God (Matt. 15:18-20; 1 Cor. 6:9-10). We believe that God offers redemption and restoration to all who confess and forsake their sin, seeking His mercy and forgiveness through Jesus Christ (Acts 3:19-21; Rom 10:9-10; 1 Cor 6:9-11). We believe that every person must be afforded compassion, love, kindness, respect, and dignity (Mark 12:28-31; Luke 6:31). Hateful and harassing behavior or attitudes directed toward any individual are to be repudiated and are not in accord with Scripture nor the doctrines of the ministry. About the Sanctity of Human Life: We believe that all human life is sacred and created by God in His image. Human life is of inestimable worth in all its dimensions, including pre-born babies, the aged, the physically or mentally challenged, and every other stage or condition from conception through natural death. We are therefore called to defend, protect, and value all human life (Ps. 139). About Final Authority for Matters of Belief and Conduct: The statements of faith do not exhaust the extent of our beliefs. The Bible itself, as the inspired and infallible Word of God that speaks with final authority concerning truth, morality, and the proper conduct of mankind, is the sole and final source of all that we believe. For purposes of the United Pentecostal Church Oklahoma District’s faith, doctrine, practice, policy, and discipline, our District Superintendent is the United Pentecostal Church Oklahoma District’s final interpretive authority on the Bible’s application.

Participant Waiver and Liability Release
Please read carefully. This Agreement affects your legal rights and is legally binding. By signing this Agreement, you are releasing the United Pentecostal Church International (UPCI), The Oklahoma District of the UPCI, The Oklahoma District Campground, The Oklahoma Children’s Ministry of the UPCI, the Oklahoma Youth Department of the UPCI, and all pastors, ministers, officers, directors, employees, volunteers, agents, representatives, and insurers (hereinafter the “Oklahoma District”) from all liability and forever giving up any claims thereof.
General - I acknowledge and agree that there are risks inherent in attendance and participation in the Oklahoma District camp, which may result in various types of injury or damage, including but not limited to, the following: sickness, exposure to infectious/communicable disease, bodily injury, death, emotional injury, personal injury, property damage, and financial damage. In consideration for mine and/or a minor child’s opportunity to attend and/or participate in the Oklahoma District camp, I, on behalf of myself and/or the minor child, acknowledge and accept the risks associated with participation and attendance. On behalf of myself and/or a minor child, I knowingly accept personal financial responsibility for any injury or other loss sustained during the attendance and participation in the Oklahoma District camp. I further accept personal financial responsibility for any medical treatment rendered to the participant that is authorized by the Oklahoma district, its agents, employees, volunteers, or other representatives. Further, I hereby release the Oklahoma District of all liability for any property damage, injury, loss, illness, or death. I further agree to indemnify, defend, and hold harmless the Oklahoma District, its employees, leaders, owners, agents, and representatives from any and all causes of action, claims, demands, losses, or costs of any nature arising out of or in any way related to my or the minor child’s attendance or participation in the Oklahoma District Camp, including but in no way limited to damages or liability from damage to property, bodily injury, personal injury, emotional injury, illness, permanent disability, and/or death, as well as medical expenses and other costs for myself or the minor child.

Consent and Certification
The information contained in this application is correct to the best of my knowledge. I authorize any reference or churches listed in the application to provide any information (including opinions) that they may have regarding my character and suitability for youth work. I understand a criminal record check may be conducted as a result of this application. In consideration of the receipt and evaluation of this application by the Oklahoma District Youth Department, I hereby release any individual, church, youth organization, charity, employer, reference, or any other person or organization, including record custodians, both collectively or individually, from any and all liability for damages of whatever kind of nature which may at the time result to me, my heirs, my family, on account of compliance or any attempts to comply, with this authorization. I waive any right that I may have to inspect any information provided about me by any person or organization identified by me in this application. Individuals using the UPCOK facilities agree to submit any legal dispute with the UPCOK for mediation before a mutually agreed-upon mediator, or if none can be agreed upon, one selected by Peacemaker Ministries. Lawsuits between believers, or threats of lawsuits between believers, are a matter of grave concern for the church, are contrary to biblical and church teaching, and mediation is an effort to resolve disputes in a biblical fashion (1 Cor. 6: 1-7). Mediation will be governed by the Rules of Procedure for Christian Conciliation of the Institute for Christian Conciliation (ICC Rules), unless modified as stipulated by the parties. In particular, subject to the more detailed provisions of the ICC Rules, mediator(s) will attempt to assist us in reaching a voluntary settlement of any disputes through mediation. The confidentiality of the mediation process will be protected and these matters will not be discussed with people who do not have a necessary interest in them. If settlement can be agreed upon, the conciliators may, at their discretion, issue an advisory opinion. Neither the opinion, nor any communications exchanged in the mediation process, will be admissible for any purpose in any subsequent legal proceeding. I also understand that my personal insurance (if applicable) is primary coverage and the Youth Camp’s insurance is secondary. I agree to and hereby release from all liability, personal and/or property, and hold harmless the Oklahoma District U.P.C., its subsidiaries, trustees, employees, agents, sponsors and volunteers from all legal responsibility, including claims, demands and lawsuits resulting from or related to any incidents arising from or connected in any manner with the Oklahoma District U.P.C. Youth Camp, including, but not limited to, liability, damages and legal fees or costs caused by or related to the negligence or intentional act of the Oklahoma District U.P.C., its subsidiaries, its trustees, agents, sponsors or volunteers. Should my application be accepted, I agree to be bound by the Bylaws and Youth Camp Policies of the Oklahoma District United Pentecostal Church, to comply with all Youth Camp Regulations and fulfill my job descriptions given by the Youth Camp Executive Committee and refrain from unscriptural conduct in the performance of my services on behalf of the district, I further state that I HAVE CAREFULLY READ THE FOREGOING RELEASE AND KNOW THE CONTENTS THEREOF AND I SIGN THIS RELEASE AS MY OWN FREE ACT. This is a legally binding agreement, which I have read and understand.
